Help us manage your side effect report in the best way by providing as much information as possible:
- Anonymized information about the patient who experienced the side effect, such as gender and age
- Contact information for the person reporting the information
- Description of the possible side effect, such as perceived signs and symptoms, date when the side effect started and the result.
